Our Mobile Medical & Dental Van brings state-of-the-art medical, dental, behavioral health, and support services directly into the community. By visiting locations such as parks, libraries, neighborhood centers, and shelters throughout Hartford, we ensure patients can receive care close to home. 

Services Available Through Our Mobile Program

Primary Medical Care

  • Routine check-ups and physical exams
  • Treatment for common illnesses (colds, flu, minor injuries)
  • Blood pressure and diabetes monitoring

  • Transition to long-term primary care
