A contribution by cash, check, or online donation is a convenient, tax-deductible way to support Charter Oak Health Center’s mission. Gifts can be made at any time of year as a one-time gift or as a recurring contribution to support the clinic and its programs and services.

Charter Oak Health Center is a Federally Qualified Health Center (FQHC) that promotes healthier communities by providing quality, safe, and patient-centered health services in medically underserved areas, regardless of ability to pay. Your donation helps us achieve that mission and better serve our patients.

Matching Corporate Gifts

Matching gifts from your employer can double or even triple the impact of your contribution. 

How to Make a Matching Gift

  • Check with your employer’s Human Resources Department to see if they offer a matching gift program.
  • Obtain and complete your employer’s matching gift form.
  • Send the completed form to Charter Oak Health Center along with your donation.
  • We will fill out the organization portion and return it to your employer.
