Coverage Enrollment Help
Medicaid Enrollment Assistance
We help patients determine Medicaid eligibility and assist with:
- New applications
- Renewals
- Screening based on income, household size, and residency
You do not need to live in a specific city, county, or area to receive our services.
Access Health CT Applications
We assist patients with applying for or updating coverage through Access Health CT by:
- Determining eligibility for health insurance and financial subsidies
- Completing or updating applications through the state marketplace
Good Faith Estimate
You have the right to receive a Good Faith Estimate explaining how much your medical, dental, or mental health care will cost.
Ask our billing department for an estimate, or request one when making an appointment.
Fees and Affordability
Charter Oak Health Center clinics are not free, but we work hard to make care affordable.
- If you are uninsured, fees are based on the sliding fee scale
- Fees are determined using Federal Poverty Level guidelines, family size, and income
- This screening process may also identify whether you or a family member qualifies for additional assistance programs

Charter Oak Health Center is committed to providing care for everyone. We welcome all patients and will never turn you away based on your ability to pay.
What to Bring (If You Are Uninsured)
Please bring the following to your registration appointment for all household members (if applicable):
- Photo identification (driver’s license, school, military, consular ID, etc.)
- Social security card(s)
- Current insurance and/or Medicaid card(s) (if you have them)
- Proof of citizenship (birth certificate, passport, certificate of naturalization, etc.)
- Birth certificate
- W4 most recent paychecks for income verification
- Proof of address dated within the last 60 days (utility bill, phone bill, lease, etc.)
- Proof of income for all household members dated within the last 60 days, such as:
- At least two check stubs dated within the last 60 days
- Letter from employer on letterhead
- Social security award letter
- Most current tax returns (if self-employed)
- Child support
- Unemployment
- Letter of support + supporter’s ID and contact information
Household members include: spouse, common-law spouse, live-in boyfriend/girlfriend.

Sliding Fee Discount Program
If you are uninsured or underinsured, you may qualify for discounted fees through our Sliding Fee Discount Program.
- Discounts are based on family size and household income
- Available for patients with incomes at or below 200% of the Federal Poverty Guidelines (updated annually)
- Sliding fee discounts apply to services within Charter Oak’s Federal 330 grant scope of service
