About This Location
Charter Oak Health Center provides onsite shelter-based services at ImmaCare to support individuals experiencing housing insecurity. Care is delivered directly within the shelter to reduce barriers to access and connect clients with essential health resources.
Our team works closely with shelter staff to coordinate care and provide referrals to Charter Oak clinical locations when additional services are needed.
