Who can receive care at Charter Oak Health Center?

Charter Oak Health Center welcomes all patients, regardless of income, insurance status, employment, residency, or ability to pay.

No. You do not need health insurance to receive care. We provide services to both insured and uninsured patients.

We offer comprehensive primary care services, including medical, behavioral health, dental, and preventive care for patients of all ages. (Services may vary by location.)

Becoming a patient is easy. You can register online, call us, or visit one of our locations. Our staff is happy to help you get started.

Appointments can be requested online, through the patient portal, or by calling your clinic location. Same‑day or urgent appointments may be available. 

After regular clinic hours, patients can follow our after‑hours care instructions to reach clinical guidance when medical concerns cannot wait.

Charter Oak clinics are not free, but we offer a sliding fee discount program based on income and family size. Fees are determined using Federal Poverty Level guidelines to help make care affordable. 

The sliding fee scale offers discounted services for eligible patients. Our staff will help you:

  • Review household income and family size
  • Complete the application
  • Determine your discount level

Yes. We provide Medicaid enrollment assistance and help with Access Health CT applications, including renewals, eligibility screening, and subsidy determination.

You can access medical records, view test results, request appointments, and pay your bill through our secure patient portal. Our team is also available to answer billing questions or set up payment arrangements.

Yes. Charter Oak Health Center is committed to providing care that is respectful and understandable for all patients. We offer language assistance services, including interpreter support, at no cost to patients. If you need care in a language other than English, please let us know when scheduling your appointment or upon arrival.

Yes. Parking is available at all Charter Oak Health Center locations. Availability and parking options may vary by site, including on‑site parking or nearby street parking. If you have questions about parking for a specific location or need accessibility accommodations, please contact the clinic before your visit and our staff will be happy to assist you.
