At Charter Oak Health Center, our Behavioral Health Division provides comprehensive mental health services designed to support individuals and families through life’s challenges. Our licensed clinicians offer care that is compassionate, culturally sensitive, and tailored to each patient’s unique needs.
We proudly serve patients ages 3 and up, helping our community access high-quality therapy, counseling, and medication management in a safe and supportive environment.
Walk-In Intake Hours
- Open Access Intake Schedule
Monday – Thursday: 8:30 AM – 2:00 PM
Intakes are completed in the order of arrival.
Please plan for a 2 hour visit once in with the therapist.
What We Offer
Individual Therapy
One-on-one sessions focused on managing stress, depression, anxiety, trauma, grief, and other personal challenges.
Group Therapy
Supportive group settings led by licensed clinicians to build connection, coping skills, and emotional resilience.
Couples Counseling
Relationship-focused therapy to improve communication, strengthen partnerships, and resolve conflict.
Family Therapy
Sessions designed to support families through transitions, behavioral issues, communication challenges, and emotional stressors.
Medication Management
Psychiatric evaluations, ongoing medication monitoring, and coordination of care with your primary care provider.
Addiction & Recovery Services
Support for substance use concerns, relapse prevention, harm reduction, and referrals to specialized recovery programs.
What to Expect During Intake
When you access Behavioral Health Services at Charter Oak Health Center, you will begin with a walk-in intake appointment. During this visit:
Case Management
Our patients tend to have many issues and challenges they face every day. Part of providing for their well-being is to provide enabling services that are designed to help them get the services they need to lead healthy lives. Our team of case managers helps our patients meet their health needs by providing individual assistance and support to ensure they have the connections they need to attain their best health outcomes.
Outreach And Eligibility Assistance
Our Outreach and Eligibility Coordinators work within the community to help current and potential patients attain the medical care they need and apply for insurance when they are eligible. The Coordinators help patients through the entire application process whether it is for Medicaid or the new insurance Marketplace.
Patient Advocate
We always strive to provide every patient with a signature Charter Oak Experience and we encourage our patients to give us their feedback. We want to hear complaints so that we can ensure that a patients experience is a good one.
