History

Since 1978, Charter Oak Health Center, Inc., a federally qualified, freestanding, Joint Commission-accredited, nonprofit community health center – has continuously provided community-based primary health care to residents in the greater Hartford area. We have grown from a small organization operating out of an apartment building to the 60,000 square foot building our patients now call their medical home.

Urgent Care – Open 7 Days a Week

Charter Oak Health Center’s Urgent Care Department is open seven days per week to provide prompt, compassionate care for patients with immediate medical needs. Our providers understand that each patient’s situation is unique, and we offer individualized attention without the long wait times of an emergency room.

Behavioral Health

At Charter Oak Health Center, our Behavioral Health Division provides comprehensive mental health services designed to support individuals and families through life’s challenges.

Pediatrics

At Charter Oak Health Center, our Pediatric Department is dedicated to providing compassionate, high-quality care to infants, children, and adolescents.

Women’s Health

At Charter Oak Health Center, our Women’s Health Department is committed to providing compassionate, personalized, and comprehensive care for women at every stage of life—from adolescence through post-menopause.

Charter Oak Health Center has received the Gold Seal of Approval by the Joint Commission, an independent, not-for-profit organization. The Joint Commission accredits and certifies more than 20,000 health care organizations and programs in the United States. Joint Commission accreditation and certification is recognized nationwide as a symbol of quality that reflects an organizations commitment to meeting certain performance standards.

Federal Tort Claims Act 

Charter Oak Health Center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Case Management

Our patients tend to have many issues and challenges they face every day. Part of providing for their well-being is to provide enabling services that are designed to help them get the services they need to lead healthy lives. Our team of case managers helps our patients meet their health needs by providing individual assistance and support to ensure they have the connections they need to attain their best health outcomes.

Outreach And Eligibility Assistance

Our Outreach and Eligibility Coordinators work within the community to help current and potential patients attain the medical care they need and apply for insurance when they are eligible. The Coordinators help patients through the entire application process whether it is for Medicaid or the new insurance Marketplace.

Patient Advocate

We always strive to provide every patient with a signature Charter Oak Experience and we encourage our patients to give us their feedback. We want to hear complaints so that we can ensure that a patients experience is a good one.
