Charter Oak Health Center works with current and potential patients to schedule appointments, apply for insurance and connect them to community resources. Our Case Management services include but are not limited to:

• Access Health CT
• Healthy Start
• PCMH+
• Sliding Fee Scale Application Assistance

Case Management services are available by appointment or on a walk-in basis.
