Our History
Since 1978, Charter Oak Health Center, Inc., a federally qualified, freestanding, Joint Commission-accredited, nonprofit community health center – has continuously provided community-based primary health care to residents in the greater Hartford area. We have grown from a small organization operating out of an apartment building to the 60,000 square foot building our patients now call their medical home. Learn more.

Joint Commission
Charter Oak Health Center has received the Gold Seal of Approval by the Joint Commission, an independent, not-for-profit organization. The Joint Commission accredits and certifies more than 20,000 health care organizations and programs in the United States. Joint Commission accreditation and certification is recognized nationwide as a symbol of quality that reflects an organizations commitment to meeting certain performance standards.
Federal Tort Claims Act
Charter Oak Health Center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
