Safety is our Priority
Charter Oak Health Center has enacted extra precautions and guidelines to maintain the highest level of safety for our patients, visitors, and employees. Some of these measures include:

Symptom screening and temperature checks at all our entrances.

Social distancing measures in waiting rooms, hallways, and all common areas.

Thoroughly cleaning exam rooms pre- and post-patient visit.

Ensuring all our staff and providers wear appropriate personal protective equipment (PPE) and follow CDC guidelines.

Requiring patients to wear masks at all times.

Eligibility
Current Eligibility: Effective April 1, 2021, those currently eligible for the COVID-19 vaccine per state guidelines include individuals living or working in Connecticut who are:

16 years of age and older.

Educators and Childcare Providers: Pre-K through Grade 12 teachers, paraprofessionals, custodial staff, food service providers, school bus drivers and childcare providers as well as in-school administrative staff. Does not include individuals who are not required to work on-site in a school.

Residents and Staff of Select Congregate Settings: Staff and residents of halfway homes, inpatient mental health facilities, correction facilities, homeless shelters, domestic violence shelters, substance use and residential treatment facilities along with others.

Long-Term Care Facility Residents: Adults who reside in facilities that provide a range of services, including medical and personal care to persons who are unable to live independently.

Medical First Responders: Individuals who respond to medical 911 calls and face risk of exposure to COVID-19.

Health Care Personnel: Individuals serving in health care settings who are at risk of direct or indirect exposure to patients or infectious materials.
Moving Forward: The State has indicated its intention to roll-out vaccine eligibility moving forward pursuant to the following plan. These are tentative plans and are subject to change by the State.
*Note: Proof of eligibility is needed at time of vaccination. This may include employment ID badge, payroll stub, business card, employer letter, other documentation, or proof of date of birth/age.
