Renovated 32 Grand Street location
February 4, 2020HARTFORD – Charter Oak Health Center, Inc. has renovated its 32 Grand Street location to provide specialty health care services beginning February 3rd.
The state-of-the-art Specialty Healthcare facility is a vital component towards the provision of high quality health care services. Specialty care has been difficult to access for the underserved community; to that end, Charter Oak hopes to improve overall health outcomes by providing access to services to our patients in a location that is easily accessible. Specialty services will include: chiropractic care, podiatry, pain management and cardiology. In addition to specialty services, enhanced care coordination will be provided to our most chronically ill patients through the utilization of Community Health Workers.
The hours of operation will be Monday – Friday from 8:30am to 5:00pm and Saturday 8:30am – 2:30pm. To schedule an appointment please call 860-550-7500. Charter Oak Health Center, Inc. provides services to patients regardless of their ability to pay.
